- Assisted Living – Seniors at Cobblestone Assisted Living Apartments who require more assistance with activities of daily living will appreciate the support of our assisted living staff. Individuals may select from a variety of specialized services. These services become a part of a tailored care plan that supports our residents as needed while allowing them to enjoy the greatest degree of independence. From educational sessions, crafts, and musical entertainment to shopping trips we will keep you busy while maintaining your interest of being independent.
- Skilled Nursing Facility –Avon Health and Rehab provides short-term, sub acute care and long term skilled nursing care for those who need help attaining or maintaining their highest physical, social, and emotional well-being.
