  • Independent Living Pebble Brook Patio Homes were designed for seniors who are still able to live independently.  Pebble Brook offers spacious one or two luxurious bedrooms homes.  Large covered patios are ideal for enjoying tea with a neighbor while listening to the sounds of nature from the village’s private woods.  Find a walking partner and stroll around the campus for excise and fellowship.  You are free to enjoy your privacy, or be as socially engaged as you wish.  Come by for a visit, talk to our residents and see why The Village of Avon’s lifestyle is truly unique.
  • Assisted Living – Seniors at Cobblestone Assisted Living Apartments who require more assistance with activities of daily living will appreciate the support of our assisted living staff.  Individuals may select from a variety of specialized services.  These services become a part of a tailored care plan that supports our residents as needed while allowing them to enjoy the greatest degree of independence.  From educational sessions, crafts, and musical entertainment to shopping trips we will keep you busy while maintaining your interest of being independent.
  • Skilled Nursing FacilityAvon Health and Rehab provides short-term, sub acute care and long term skilled nursing care for those who need help attaining or maintaining their highest physical, social, and emotional well-being.  In addition to these services Avon offers a specialized care community called Touchstone Terrace for seniors facing the unique needs of dementia and Alzheimer’s.   

    Other medical services on campus include physician, and psychiatric consultation, podiatry, dental care, pain management and emergency services. Physical,
    occupational and speech therapists, nurses, dietitians, pharmacists, social workers, activity therapists, hospice staff and case managers work closely with the patient and family to achieve a common goal of maximum dignity, function and well-being.  This goal is to renew life’s special moments through a continuum of care in an atmosphere of “being home with friends”.
